After reducing her weight from 287 to 146 pounds, this accomplished female patient in her mid-30s was left with loose skin and excess tissue extending from the upper thighs toward the knees. Her remarkable 141-pound weight loss had transformed her body, but the remaining skin continued to add width, folds, and heaviness to her legs.
Her concerns included:
Inner-thigh laxity can occur in two directions. Vertical laxity refers to tissues that have sagged downward and may be improved with an inner thigh lift using an incision placed within the groin crease. Horizontal, or circumferential, laxity refers to excess around the width of the thigh and requires tissue removal along the length of the inner leg.
Because this patient had both patterns, including substantial circumferential excess extending toward the knees, a groin-incision inner thigh lift alone would not have provided enough correction. Dr. Agha therefore recommended a Vertical Thigh Lift, also known as a Thigh Reduction. Although the incision runs vertically, its purpose is to reduce the width and girth of the thighs from the groin toward the knees.
Thighplasty is one of the least commonly performed areas of body contouring. A typical plastic surgeon performs approximately 1.2 thighplasty procedures per year, while Dr. Agha performs more than 36 thigh procedures annually. His extensive experience with different patterns of thigh laxity allows him to determine whether a patient needs an inner thigh lift, a Vertical Thigh Lift, or another form of thigh contouring.
Dr. Agha first used targeted liposuction to refine the upper inner thighs, saddlebags, and areas above the knees. Each leg was treated according to its individual shape, including additional contouring above the right knee to address her natural asymmetry.
He then removed the long sections of excess skin along the inner thighs through incisions extending from the groin toward the knees.
The after photos show a dramatic reduction in thigh width and inner-leg excess. Her legs appear slimmer, straighter, and better proportioned from the upper thighs through the knees. The heavy inner-thigh folds have been removed, the saddlebags appear more refined, and the skin above the knees rests more smoothly.
