After reducing her weight from 287 to 146 pounds, this accomplished female patient in her mid-30s began addressing the loose tissues left behind by her remarkable 141-pound weight loss.
Her first stage with Dr. Agha was a Vertical Thigh Lift, also known as a Thigh Reduction. That procedure removed circumferential excess along the inner thighs, creating a slimmer contour from the groin toward the knees.
Approximately four months later, she returned to address the waist, lower back, pubic area, outer and front thighs, hips, and buttocks.
She had also undergone a previous tummy tuck in Iran. Although her abdomen remained relatively flat, she was left with an existing lower-abdominal scar, residual looseness, and an uneven transition into the surrounding areas.

Dr. Agha designed a High-Definition Lower Body Lift™ around her anatomy, previous tummy tuck, and recent Thigh Reduction. The frontal portion revised her existing abdominal scar, removed the remaining lower-abdominal excess, and lifted the pubic area as part of the continuous lower-body correction.
The front thighs were elevated and secured to strong supportive tissue near the pubic bone, while the outer thighs were lifted to reduce the downward pull affecting the sides of her lower body. The monsplasty improved the sagging and bulging pubic contour, creating a smoother transition between the lower abdomen and upper thighs.
From the back, Dr. Agha lifted the buttocks while preserving tissue from the lower back for natural augmentation. Rather than discarding this tissue, he folded and positioned it beneath the buttocks to rebuild shape using the patient’s own body.
He then transferred 240 cc of purified fat to each buttock, adding fullness through the central and lower regions without implants. Liposuction of the lower back, waist, flanks, and saddlebags further refined her silhouette and was adjusted to address her natural asymmetry.
A typical plastic surgeon performs approximately one lower body lift per year, while Dr. Agha averages 27 lower body lifts and revisions annually. The after photos reveal a narrower and more defined waist, smoother transitions through the lower back and hips, and a cleaner lower-abdominal contour. Her previous tummy tuck scar has been revised, while the pubic area now appears lifted, smoother, and better proportioned to the surrounding anatomy.
From behind, the buttocks appear elevated and fuller rather than flattened by the lift. The outer thighs also sit higher, creating a more graceful line from the waist through the hips and legs.
Her two procedures addressed different dimensions of post-weight-loss laxity. The Vertical Thigh Lift reduced the width and excess skin of the inner thighs, while the High-Definition Lower Body Lift™ lifted and reshaped the waist, lower back, outer thighs, pubic area, and buttocks.
