SMAS Facelift
NEWPORT BEACH, CA

The Gold Standard in Facial Rejuvenation

Newport Beach SMAS Facelift at The Aesthetics Center

Non-surgical treatments have come a long way. Threads, lasers, energy devices, and injectables can all address early or mild signs of facial aging with minimal downtime. But there is a ceiling to what any non-surgical treatment can achieve. When the structural changes of aging are real and significant, surgery remains the only solution that addresses them at the source.

The most effective facelift techniques all work by addressing the same underlying layer: the SMAS, or superficial musculoaponeurotic system. This is the connective tissue and muscle layer that lies beneath the facial skin. With age, the SMAS weakens and descends, dragging the overlying skin with it and producing the jowling, sagging, and deep creasing that defines an aging face. At The One Plastic Surgery Center in Newport Beach, Dr. Agha is an expert in the full range of SMAS facelift techniques and selects the right approach for each patient's anatomy and goals.

WHY CHOOSE DR. AGHA FOR YOUR SMAS FACELIFT?

The SMAS facelift is one of the most commonly performed facelift techniques in the world, but the quality of results varies enormously depending on who performs it. The difference between a facelift that looks natural and one that looks operated on comes down to surgical judgment, technique, and an understanding of how the face ages in three dimensions, not just two.

Dr. Agha completed his MD and PhD at the University of Cambridge on a Wellcome Trust scholarship and his seven-year plastic surgery residency at the University of Pittsburgh Medical Center, one of the most respected programs in the country. He trained directly under Dr. Bryan Mendelson, a globally recognized authority on facial anatomy and facelift surgery, giving him a foundational understanding of the SMAS and deeper facial structures that few surgeons acquire through standard training alone.

Dr. Agha performs the full range of SMAS techniques, selecting the most appropriate approach based on each patient's unique anatomy and goals. His three-dimensional philosophy means he approaches every facelift not as a tightening exercise but as a structural restoration, rebuilding the definition and natural contour of a younger face. The result is a patient who looks genuinely refreshed, not surgically altered.

What is an SMAS Facelift?

Are you bothered by a softening jawline, jowling, or sagging skin that makes you look older than you feel? The SMAS facelift, also known as a rhytidectomy, may be exactly what you are looking for. This surgical procedure targets the lower two-thirds of the face and neck, directly addressing the structural causes of facial aging rather than just tightening the surface. It effectively treats:

  • Jowling and loss of jawline definition
  • Double chin and excess submental fat
  • Sagging facial skin and tissue descent
  • Deep nasolabial folds and facial creases

By tightening the SMAS layer and repositioning the deeper tissues of the face, the SMAS facelift restores the natural contours of youth, leaving patients looking genuinely refreshed rather than merely tighter. It is one of the most widely performed and well-established facelift techniques available, with a strong track record of long-lasting, natural-looking results.

Understanding the Main Types of SMAS Facelift Procedures

The SMAS layer can be manipulated in several different ways, each with its own advantages depending on the patient's anatomy, degree of aging, and treatment goals. Broadly speaking, SMAS facelift techniques fall into three main categories. All are highly adaptable to individual needs, and Dr. Agha selects the most appropriate approach during your personal consultation.

SMAS techniques are particularly well-suited for patients experiencing significant volume loss and sagging in the lower face and neck. Where improvement in the upper cheek area is also needed, fat grafting can be incorporated into the same procedure to address this limitation.

SMAS Plication Facelift

The SMAS plication facelift tightens the SMAS layer using sutures, without detaching it from the underlying tissues. The SMAS is folded on itself and secured with permanent or long-lasting dissolvable sutures, lifting the deeper facial tissues and reducing sagging. Many variations of this technique exist, each with its own suturing pattern and outcome. One of the most widely known is the MACS facelift (Minimal Access Cranial Suspension facelift). Others include lifestyle facelifts, lunchtime facelifts, and quicklift facelifts.

During the procedure, the facial skin is lifted to expose the SMAS layer. The SMAS is then folded and sutured according to the planned pattern. Excess skin is removed and the incisions are carefully closed.

SMAS Lift Facelift

In a standard SMAS lift, the SMAS layer is not just folded but fully detached from the underlying tissues, trimmed, and repositioned at a higher position. This allows for a more thorough correction than plication alone, with greater control over the direction and degree of lift. Because the SMAS musculature can be elevated in varying directions, the SMAS lift facelift is highly customizable to each patient's facial anatomy and provides long-lasting, natural-looking results.

Lateral SMASectomy

The lateral SMASectomy addresses signs of aging by surgically removing a precisely defined segment of the SMAS layer, rather than simply tightening or repositioning it. After the skin incision is made and the tissue is carefully elevated, the surgeon assesses the excess SMAS tissue present. The redundant SMAS is excised, and in some cases, portions may be repositioned to other areas of the face. This approach offers a more precise and well-defined outcome compared to SMAS plication, particularly for patients with more pronounced tissue excess.

What Are The Key Differences Between SMAS Facelift and Deep Plane Facelift Techniques?

Patients researching facelift options often encounter both the SMAS facelift and the deep plane facelift and wonder how they differ. The distinction comes down to how the skin and SMAS are treated relative to each other. In an SMAS facelift, the facial skin and the SMAS layer are addressed as separate components. In a deep plane facelift, the skin and SMAS are elevated together as a single composite unit, at a deeper tissue level beneath the facial muscles.

Proponents of the deep plane technique argue that it offers superior correction of the upper cheek and smile lines, better skin circulation during healing, and a potentially faster recovery due to less extensive skin undermining. However, the SMAS facelift remains one of the most widely performed and effective facelift techniques in the world, with excellent long-term outcomes for the right patient. Dr. Agha performs both and will guide you toward the approach that best suits your anatomy and goals.

What are the benefits of an SMAS Facelift?

The SMAS facelift offers a compelling combination of meaningful results, established safety, and relatively manageable recovery. Because it targets the deeper structural layer of the face rather than just the skin, outcomes are more natural and longer-lasting than surface-level tightening alone. Many patients report looking and feeling 10 to 15 years younger after surgery, with results that hold for a decade or more.

The SMAS facelift also works seamlessly alongside complementary procedures. Brow lifts, eyelid surgery, fat grafting, CO2 laser resurfacing, and Botox can all be combined in the same surgical session for a more comprehensive facial rejuvenation. This makes it an ideal anchor procedure for patients looking to address multiple areas of facial aging in a single recovery.

How Is a SMAS Facelift Performed?

SMAS facelift surgery is performed under general anesthesia or local anesthesia with IV sedation, depending on the extent of the procedure and patient preference. Incisions are placed within the natural creases in front of and behind the ear, often extending slightly into the hairline, where they heal with minimal visibility.

The facial skin is carefully elevated to expose the SMAS layer, which is then tightened, repositioned, or partially excised depending on the technique selected. Because the lift is supported by the stronger SMAS layer rather than the skin alone, results are more durable and look more natural than skin-only tightening. Excess skin is then trimmed and the incisions are closed with sutures.

Where appropriate, the malar fat pad, the superficial fat compartment of the midface, can be repositioned upward during the same procedure using the extended SMAS technique. This restores lost cheek volume and enhances the malar prominence, contributing to a fuller and more youthful midface.

A neck lift is commonly performed alongside the SMAS facelift for comprehensive lower face and neck rejuvenation. Additional optional enhancements include eyelid surgery, fat grafting, deep wrinkle fillers, brow lift, and Botox, which can all be combined in a single procedure.

SMAS Facelift Recovery and Aftercare

Most SMAS facelift patients can expect a recovery period of approximately two to three weeks. In the first days after surgery, bruising and swelling are normal and expected, and these resolve gradually over the following two weeks. Most patients feel comfortable returning to work and resuming light daily activities by the two to three week mark. Strenuous exercise and vigorous activity should be avoided until the surgical wounds are fully healed.

To support optimal healing and maintain long-term results, patients are advised to avoid smoking, limit sun exposure, keep incision sites clean and moisturized, and attend all follow-up appointments. A consistent skincare routine with daily broad-spectrum sun protection is one of the most effective ways to preserve your facelift results over time.

SMAS Facelift Results

The SMAS facelift produces long-lasting, natural-looking results by lifting and tightening the deeper structural tissues of the face, smoothing wrinkles, eliminating jowling, and restoring facial contour. Initial improvement is visible within the first days after surgery as swelling begins to resolve, with final results fully apparent at around six months.

Results typically last ten years or more, and patients consistently report that the improvement to their confidence and overall well-being is just as meaningful as the physical change. The key to maintaining your results long-term is diligent sun protection, a stable weight, and a consistent skincare routine.
