Comprehensive Lower Body Lift Revision with Inner Thigh Lift & Scar Revision, Buttock Augmentation - October 2026

SURGERY INFORMATION:

LEARN MORE ABOUT: High-Definition Lower Body Lift™, Buttock Augmentation, Thighplasty

PATIENT AGE: Early 40s

SURGEON: Dr. Siamak Agha MD, PhD, FACS

SURGERY PLAN TRANSCRIPT:

So today we are on our patient's second stage of corrective surgeries. During the previous one, we fixed her breasts. We took implants out, right? Yeah. Yeah, implants out. We did a breast lift, we did a pocket correction, and we put smaller implants. This one is just a little bit lower, so I'm going to actually put a suture here and fix that today, and then the breasts are done and they look beautiful.

But her lower body work that was done elsewhere is a complete mess, to say the least. So she has had a fleur-de-lis tummy tuck. This is her tummy tuck incision, and I'll show you on the back. It goes really high up, and then someone had the audacity to cut her here for an inner thigh lift. And I'm seeing a lot of these on the internet, and they're terrible surgeries. And you see why, because why would you even do this surgery? Why would you put a scar that is extremely noticeable on somebody when there is a better technique?

And the technique would be to place it in the groin crease and reconstruct the groin crease. Why? Laziness, number one. Number two, because this is a harder procedure to perform. You can get labial spreading with it, you can get scar migration with it, so you have to do it correctly. This is easy. Even if it migrates, well, it was already there in the first place. It was already visible. So if it migrates, no problem. But regardless, it's a terrible surgery.

And I wish people would understand what they are getting themselves into, because sometimes when you're excited after all the weight loss and there is a solution, you may become so excited that you proceed with the solution without really understanding the full implications of what you are getting yourself into. And at some stage, you'll regret that decision if it's not done correctly.

Anyhow, so we have one incision here and we have one incision here. We have, obviously, the fleur-de-lis tummy tuck incision, and normally I can fix these. But this one, and I've already told her I can't fix it. There is such a big gap. There is no way for me to be able to take this from here to here. We are talking about two inches of missing skin. So we'll see what we can do.

We're going to lift the front of the thighs. I'm going to pull this in, and we'll find out how much we can pull it in. Our surgery is not fixing this. It's fixing the front of the thighs, the outer thighs, the tummy, the backside. But I'm going to try to help her with this, and then we'll come back and see if we can do more for her at that time or not.

On the backside... Keep on rotating, sweetheart. On the backside, we are going to do a scar revision on this widened scar. And then what I wanted to show you is how they cut her up all the way up here. So we got to take that out and perform a significant buttock lift for her. We are talking about, look at that, this much skin being removed. More than half of her butt skin will be removed, and then this fat will be utilized as a pad.

When it's used as a rotational flap, it is 100% viable, and it's not like fat transfer. We will do some smaller fat transfer in addition.

Surgery Results - Back

TRANSCRIPT

So this is obviously our buttock lift. What you notice is that we performed a substantial lift of the whole buttock unit, removing about 50% of her buttock skin. Essentially from here to there was cut out, and all of that fat residing under the skin used to augment her buttocks with. These incisions here are from her previous inner thigh lift, the area that I'm trying to fix from the front for her, and hopefully I will be able to do that for her. This is her side profile. Nice, beautiful round buttocks that we were able to create for our patient while keeping her hourglass silhouette.

So today's surgery on the front was really very challenging only because the way that her previous inner thigh incision was placed is extremely unorthodox, but unfortunately, more and more are being done this way. Her incisions were really here. So that's really, yes, it's an inner thigh incision, but it's not what is normally done in plastic surgery. So what I wanted to do is to lift as much of the front of the thighs up and cut as much of that incision as possible. I had no idea whether I could bring this inner thigh incision in or not, but I think we accomplished what I wanted to do, which is to bring it in enough to really place it hopefully in a bikini and hide it. And I think we were able to make a substantial correction of her prior incisions. You see the end of it here. This is the end of her prior. So this from here was migrated to here, and then the same thing here. You can't even see this one. This one worked much better. So a big accomplishment to say the least, and hopefully she'll be very pleased with the results and hopefully we're done.
