TRANSCRIPT

Happy Monday, everyone. Today we are performing our high definition lower body lift. Super excited for our patient. She has had previous liposuction and subsequently with obviously aging has developed lower body laxity. Today we'll be doing a comprehensive correction of her abdominal region, pubic region, thigh, and buttocks in unison. She cares a lot about the butt, so I'm gonna spend half of my time on making her an more of a rounder, perkier buttocks. So on the front we'll be doing our high definition tummy tuck, pubic lift, thigh front lift. And let me rotate you, sweetheart. Outer thigh lift like that. Look how smooth she's gonna become. And then a good amount of buttock lift, buttock repositioning, buttock augmentation with lower back fat, and correction of all these excess areas of fat and indentations that was created as a result of previous liposuction. What you'll notice also is the fact that she has had a posterior thigh incision, an incision in the infra-gluteal crease, but it hasn't been reconstructed and not enough of the posterior thigh has been removed. I'm not gonna go after that today because that needs to be done at a later stage. Once this settles and falls into the right place, we can come back and do this for her. She's also had an inner thigh lift, and that also has to be corrected. So another thing that you'll notice is the fact that I have marked her 12th rib. This is the last rib. This is essentially her rib cage, and this is her abdomen. In the process of having a breast augmentation and lift before finding us, what they have done is to lower the whole breast unit from here to here by placing a larger implant. When you use a larger implant, invariably the inframammary fold is lowered and loosened up, and you will get this problem, which is migration of the breast from the mid-chest to the lower chest. Also, the inframammary fold has not been reconstructed correctly like this. So you'll see breast skin merging into a abdominal skin, a new inframammary fold being recreated by the implant here, and the inframammary fold that should have been here being visible on the breast. To correct this completely would involve removing the implants, reconstructing the inframammary fold at a much higher place, redoing the breast lift, and replacing the implant. Having said that, she's not ready to do all of that because her breasts do actually look pretty nice. So today we are just moving the inframammary fold high enough to hide it behind the breast and doing an inframammary fold reconstruction.

So just look at the difference we've made in a few hours. This is obviously a buttock lift with buttock enhancement. Our patient desired to have fuller buttocks, and we've transferred all of her excess tissue into her buttocks in the form of a flap, which means that it comes with its own blood supply, and then some grafted fat, which is more like a Brazilian butt lift. This butt was always bigger than this one, and I think still a little bit bigger despite the fact that I put more fat here. I wish I had more fat, but I think from the front, I'm now gonna take some fat from the lower abdominal region and then add it into this area. I think this area needs a little bit more fullness. But overall, a great transformation. You'll see that the infra-gluteal creases are now even. This one was actually lower. We've elevated them through the buttock lift work as well as lifting the outer thighs.

So guys, just look at redefining our patient's abdominal contour through the high definition lower body lift tummy tuck approach. What you'll see is her definitions reflecting and shadows of the oblique muscles. Truly a sculpted look using the technique of the high definition tummy tuck. Obviously, I lifted the front of her thighs and the pubic area and coming to the inframammary creases. I had to make them tighter and crisper for her, and that was also done, um, with today's surgery. There we go.
