Happy Thursday, everyone. So today, we are starting the first surgery for our patient who is from Idaho and has lost 130 pounds. 130 pounds. And how much do you weigh now? 130. Okay, so she's lost almost 50% of her body weight, and she doesn't have much fat on her, but we have a tremendous amount of skin laxity and excess. So we'll be doing our high-definition lower body lift for her.

On the front, removing all this loose skin, tightening everything up. As we come to her sides, we're going to remove, may I put your arms, nine and a half inches of skin from the hip region. You see all of this is going to get cut and make her tight. On the back, keep on rotating. We're going to do a bra line upper body lift so that I can correct this before I do a lower body lift. Typically, I do these on different days because truly it gives patients a better result when you actually separate them. But today, if I do my lower body lift now, I'm going to capture every wrinkle within my scar, and then she's going to have this permanently, essentially, built into the scar, which I do not want. So the first thing I'm going to do is to cut the excess skin to make this smooth, and then immediately do her lower body lift, removing nine and a half inches of skin from the hip area and the buttock area, and lift everything up and augment it with her own excess tissue. You ready? I'm ready.

So my patient is about to fly back home tomorrow. She's been here for five weeks. Five weeks ago, we did a very comprehensive high-definition lower body lift on her. She's healed immensely well. She has one tiny opening, and I'm gonna show that to you. And otherwise, just... May I? Yeah.

Look how pristine the incisions are. Healing beautifully. Obviously, I'm not gonna show you the pubic area, but look at that line. Come closer. Okay. So her incision is perfectly placed at the junction between the lower abdomen and the thigh. We lifted the thighs. We removed a ton of loose skin and tissue. We did a structural lift. This is her navel. She has more swelling than a skin-only body lift. Skin removal body lifts are about removing the tissue and closing the incisions. They are much easier body lifts to perform, but they give you less correction because you're just... It's like a panniculectomy in the front. You're removing the tissue and you're closing. You're not elevating the tissue. You're not repositioning the fat. Therefore, typically with skin-only body lifts, they're easier to perform, take half the time that these type of body lifts take to perform, give you about half the correction with it, and half the recovery is needed. With these type of body lifts, these are like deep plane face lifts. You get more swelling, more pain, more bruising, more recovery, and a much better result because things have been corrected at the level of the fat, not at the level of the skin. Okay. And here you go. And this is her three-dimensional silhouette. As I said, she's got 15... She tells me she's 15 pounds over her normal weight, which means she's got 15 pounds of swelling, maybe 10 pounds. Maybe she's gained a few pounds on high-nutrition and high-protein diet, which I like because I think she can use it. But 10 pounds of this is swelling, and it will take another three months, four months for it to go away. If you remember, I was planning to cut this for her because I wanted to make sure that my incisions end up pristine. When we got doing the surgery, after this was done, this area was tight, so I decided not to do it, and her incision still looks pristine, so it really did not need to be done at this stage. And we're still gonna do it. That's gonna be her next surgery when she comes back. We are already planning to cut this area and go after her arms and her breast. But also notice that we created or restored her buttocks. You know, the buttocks had a lot of skin. They had become pancakes, meaning that they had lost their projection, flattened, and became wide and long, and we had to pull that together and fix that. Another area that will help that we were talking about today is this. The posterior, which means back, of thigh lift defines the border of the buttocks, but she's okay with it, and ultimately, this is her surgery, not my surgery. So if she's happy, I'm happy. But we're gonna put more fat in here in due course, once she gains 10 to 15 pounds for me, right? Yes. And we'll finish her surgeries at that stage. So in a nutshell, what do you think? I'm very pleased. Because it's all about you, right? Very pleased. I can't wait to see it when... even when the swelling goes down. It'll be even better. Yeah. Nice correction. Yes. I mean, a ton of correction. And I like the curves. It looks good. It looks good? It looks great. Definitely the right choice. Thank you. You're welcome. Did you wanna show that? Oh, yeah, keep on rolling. So this is her only opening, and she has got a tiny opening. This is a superficial opening. And when you are having four or five feet of incisions on you... Look at the rest of her scars, though. You see it's pristine and beautiful. Yeah. Let me just rotate here. This one is a little bit more red, but the pink will go away. So she has had no openings, and I'm super pleased. But usually I tell patients that when you have four feet of incisions on you, minor openings are the name of the game with these surgeries because there is just so much happening and we are using tiny threads to put your tissues back together. So I'm super pleased with just having one of these because you can have these minor openings. It's not a big deal. You do dressing changes, and she's gonna heal. But it comes with doing a lot of incisions, and we need to do these incisions to obviously remove the excess tissue and skin and mobilize and contour and so forth. So there you go.

So today I was planning to perform a bra line upper body lift for our patient, as well as a buttock lift. Normally, I don't do these procedures at the same time. I normally like to do the lower part first, give the patient time to heal, develop small amount of skin relaxation that typically happens with these surgeries, and then come and tighten the back again through the upper body lift incision and creating the best results for the patient. Given the amount of skin laxity that she had, I decided to do both of them at the same time today. But having said that, when I started with the buttock lift and buttock enhancement, I realized that I won't be able to cut as much from the upper back, and it may be prudent to wait and do that in the future. So here we are. We are now finished with our buttock lift and buttock enhancement and outer thigh lift, and I will be continuing with the patient's surgery on the front and in the future, we can go after the upper body another day. People often think that operating on smaller patients is easier, and that is true. It is easier to operate on smaller patients, especially if you're doing a skin-only body lift or a skin-only surgery. But our aim here is to transform and create shape and contour the patient. And in order to do that, in order to create concavity, in order to bring fullness here, in order to bring projection to the buttocks, I would need to be able to sculpt the patient to remove the excess fat and to use that to create the third dimension of profile or buttock enhancement. And I wish today we had more fat. I truly do.

We essentially took that flat abdomen and created this silhouette with definitions, elliptical navel. We performed a pubic lift and removed staggering amount of skin from her thighs in continuation with the outer thigh lift from the back. There you go.
