SURGERY PLAN TRANSCRIPT:

Happy Wednesday, everyone. I am super excited to be doing a high-definition lower body lift on our beautiful patient. And how much weight have you lost? 70 pounds. 70 pounds. And what happened prior to this? You have had surgery? Yeah, I had surgery in Europe, in Germany. It was fine, but I did not achieve what I wanted. Right. Which is why I looked for the best. Thank you. And home for you is? Eastern Europe. In Europe. Right.

So the reason she hasn't achieved what she wanted despite having a lower body lift four years ago has to do with the fact that she has had a skin removal surgery. Skin removal and lower body lift means that you cut here, you cut here, or typically a little bit higher, and then you just suture the skin together. Now, as I'm doing this, it's tight. It's already pulling down because the skin is attached to the muscle. So when you do this, over time, it's going to relax again. It's going to get pulled down. In order to achieve a better result, I do what I call structural lift. It's a terminology that I coined to describe what we do, which is lifting the whole structure up. And in order to achieve that, you have to undermine the tissues, you have to lift them up, you have to create more dissection. It's more invasive, it requires more healing, and it's a bigger surgery, but it gives you a better result. The incisions are the same, but we work harder through that incision by lifting, dissociating the fat from the muscle, lifting it up, and suturing that fat back to the muscle. So all these attachments that go from your skin to your muscle, most of them are dissociated, so we can get more elevation, a deep plane lift, and then anchoring sutures are placed in order to keep it there. Otherwise, the results relax pretty soon, and this is only four years out. So today, we'll be going after her navel. We're going to remove all her previous scar. We're going to do a structural lift. We're going to do Renuvion of the inner thighs. She really would benefit from a vertical incision here, but she doesn't want that, so we compromise because at the end of the day, it's her surgery, and she understands that Renuvion will not give her the best result, but she's okay with that. We're going to do an inner thigh lift in addition. On the back, she has these incisions, but you see I'm going to cut another 10 centimeters of tissue, reposition the fat. We're going to do a spiral thigh lift with a posterior thigh lift, Renuvion of the back. This buttock is lower than this one. We're going to adjust that and fix that. So again, four years ago, she has had a lower body lift, but look at the laxity that she already has developed. I am sure it was much tighter after, right? Yes, it was. It was indeed. So it was tight before, but it has relaxed, and it relaxes because it was a skin removal lower body lift rather than a structural lower body lift. Okay. Good luck to both myself and you. You're so cute. Thank you.

7 WEEK FOLLOW UP TRANSCRIPT:

So seven weeks out from her lower body lift redo and spiral thigh lift. She's about to fly back to Europe tomorrow, and here she is 100% healed. We did a complete lower body lift redo. We did a structural lift. Her prior lower body lift was done about four years prior to this, but you can see the difference.

She'll tell you what the difference is. I essentially made a depression of her previous navel. Her previous navel was really flat. We pushed that in and sutured that to the muscle, so we have an indentation. And as we rotate her, you can see how beautiful her backside looks. This buttock was an inch lower than the other one, so we fixed that. These are the spiral thigh lift incisions that start on the back. Again, very symmetrical, beautiful, appropriate, not overdone. Fullness and restoration of the buttocks. She's got a beautiful feminine look. And also, she had an inner thigh lift with a scar that is appropriately placed here. So in a bikini or in a thong, you'll never know that she has had a lower body lift. Obviously, she's got some discoloration as a result of reaction to the tape, but this will be gone in six months, and she's going to look pretty fabulous, to say the least. What do you think? I look pretty fabulous right now. I'm very happy with the result, and it exceeded my expectations, actually. I'm very happy because I wanted a correction, a true correction that would fulfill at least three conditions. The first would be to have a navel again, which I lost during the previous surgery. The second was to really have the exterior thighs straight. Mm-hmm. Because the prior surgery did not really correct those. And finally, to have the interior thighs without the scar, the vertical scar. Right. And all of them are achieved. And I was kind of afraid of the inner thigh lift scar, but it healed perfectly. I followed the instructions, and I stayed put, and the incision, to start with, was very well placed and done. So I'm very happy. We're very happy with you. It healed remarkably well, and now you're about to go back home. We're going to miss you. Yeah, I'm going to miss you guys, too, but I'm going to come visit. Thank you.

Surgery Results - Back

TRANSCRIPT

So here we are, and you're looking at our buttock lift and outer thigh lift redo. What you'll notice is that our patient now has a beautiful round buttocks. She already had this procedure performed in Germany about four years ago and still had a good amount of outer thigh and buttock skin excess. In reality, today, we removed about 10 centimeters of excess skin from the buttock area and outer thigh area, and then I did a complete buttock lift redo, outer thigh lift redo, removing her prior scars, liposuctioning her flanks, adding fat to the buttocks in the form of buttock fat transfer, and flap augmentation. Coming to the back of the thighs, the areas that are marked were treated with Renuvion to tighten the skin of the back of the thighs and the inner thigh, where she has a good amount of skin laxity. And then I performed the beginning of the spiral thigh lift or the posterior thigh lift, cutting her in the infra-gluteal crease, removing about one and a half to two inches of skin from that area, and lifting the back of the thighs and reconstructing the infra-gluteal crease meticulously. Another point to make is that this buttock was lower by about one inch than the other one. So in the process of performing her buttock lift redo and the posterior thigh lift, we also corrected that for her. So there you go.

Surgery Results - Front

TRANSCRIPT

So we're now finished with the surgery. On the front, I converted her flat abdominoplasty that was done previously into our signature high definition tummy tuck. We deepened her navel placement since it was very superficial, and we recreated her definitions. I next performed a pubic lift, removed her previous scar, removed about two or three centimeters of lower abdominal skin, lifted the front of the thighs in continuation with the outer thighs, and finally continued with the inner thigh lift and performed an inner thigh lift for our patient. And she looks pretty amazing, to say the least.
