SURGERY PLAN TRANSCRIPT:

So today we're doing a high-definition lower body lift on a young patient who's lost 105 pounds. What you'll notice is that her upper half actually looks pretty good, but the lower half has a substantial amount of skin laxity. So this is a person who had more of a weight loss from the lower half, meaning that her fat distribution was mainly in her lower half, and when she lost the weight, she has more skin laxity of the lower half, more than many of our patients.

On the front, we'll be performing a high-definition tummy tuck. She's going to look sculpted and amazing with pubic lift. We're going to lift the front of her thighs. And as we come to the back area, we will be doing an outer thigh lift like this and a substantial buttock lift. And what you'll notice is how the buttock area has drooped from here to here. She's got a lot of lower back and upper buttock skin laxity. Her butt is very low. Her muscles are here, but the butt is actually here, and the thighs are low. So we have to reposition this over the muscle at a higher position. And even if I cut inches and inches of skin out, you'll see that the back of the thighs do not get elevated with this procedure because this procedure is a buttock lift. It's not a posterior or spiral thigh lift. So she will need that procedure in due course, too, because today I'm setting the upper aspect of the butt correctly. I'm doing a structural buttock lift. I'm repositioning this higher. But we still need to come back and set the lower, the inferior boundary of the butt by doing a spiral thigh lift, infra-gluteal crease reconstruction and repositioning, and a posterior thigh lift at another time. If I do these procedures in combination, she will struggle with healing. So I've learned the hard way that it's best to do one first and then come back and do this one at a later stage. You ready? Mm-hmm. Okay.

3 MONTH FOLLOW UP TRANSCRIPT:

Our beautiful patient is three months from her lower body lift, and this surgery was rather comprehensive and she still has some swelling. She tells me in the morning everything is flatter, but in the afternoon she swells up. These surgeries are more comprehensive than your average skin removal body lift. It has to do with the fact that we are raising flaps, we are mobilizing fat over muscle, we are repositioning it, so there's a lot more healing that has to happen.

Just like a deep plane facelift, these surgeries are like deep plane body lifts. We are in the deeper planes. We are lifting and repositioning things, and it takes longer to heal, but also it gives you a better result. You can remove more skin, you can do a structural lift, and you can create more pleasing results. What do you think? I love it. I feel like a teenager again. Which part do you love the most, front or back? Back. Back? Yeah. I love your front, actually. Look at this tummy. It looks really insane. She's got a beautiful three-dimensional contour, very athletic. Still with some swelling. This is going to get better in three months' time. We'll do a follow-up with her, and we'll make sure that it looks amazing, but she's healing beautifully. This is her scar in the front. You see? Very low, and then it goes all the way around, and this is her backside and her buttocks. She had so much skin here. We removed a good amount of buttock skin, did a comprehensive lift. I wish I had a little bit more volume here, but again, it's not my surgery, it's her surgery. And we still have swelling here, so once the swelling goes down, we get more lower doses, which should concavity of the back, and we can always reassess. And if we need to add a little bit more volume, if she desires, we can do that. But if she's happy, which she is, then I think my job is done for now. Will you do it again? Yes. Yeah. It was worth it. It was hard, wasn't it? Yeah. Yeah. It's a hard surgery, but it's really rewarding for both of us. Thank you.

Surgery Results - Back

So you're looking at the buttock lift portion of our surgery. During the past 20 years, I have performed hundreds and hundreds of buttock lifts, and I can tell you that this buttock lift today required the most amount of lift that I've ever done. Meaning that I ended up removing a substantial amount of skin from the lower back and the buttock area, more than I've ever had to for any patients. And then I lifted the buttock unit over the muscle and sutured it at a higher position, using the excess lower back fat to augment the buttocks width, liposuction her waist area, and added that fat to the lower buttocks. And as you can appreciate, she's got a beautiful, rounded buttocks. And you'll also notice that we spent a lot of time making sure that the infra-gluteal creases are the same. Now, these creases are actually very loose, and in future, I may have to end up coming back and doing a spiral thigh lift, infra-gluteal crease reconstruction, and a posterior thigh lift for her.

So I'm now finished with our surgery and just look at our patient's abdominal contour. This is the result of her high-definition tummy tuck and completion of her lower body lift with pubic lift and thigh lift from today. And this is her pain pump, which will make her far more comfortable after this surgery.
