PATIENT AGE: Late 50s
SURGEON: Dr. Siamak Agha MD, PhD, FACS

So there you go. Already a huge change has been made. On the back, I removed between four to five inches of lower back and upper buttock skin and fat, so that would result in a buttock reduction. And obviously, our buttocks are smaller because that's what she wanted. They're also less wide. Again, that's what our patient wanted, but still have a beautiful hourglass look. Once the buttocks were lifted and reduced, we then augmented the buttocks with lower back fat flaps and additional fat transfer and created a beautiful round buttocks, as you can appreciate. Smaller, but round and beautiful. And then the back of the thighs were extremely uneven after her previous lipedema liposuction, so I lipo shifted the fatty layer to create a monotonous layer. It's much improved, as you can see, but there are areas that despite my best effort, there is still some contour irregularities. Hopefully, with the skin reduction that we have planned on the front, I can correct those areas as well. But so far so good, and now we'll be turning her and continuing the rest of the surgery on the front.
So this is a front view of our lower body lift revision. On the front, I performed a tummy tuck scar revision, lowering her tummy tuck incision, and at the same time lowering the pubic area which had been over-pulled. The navel is from her previous surgeries through the tummy tuck incision. I continued with the outer thigh lift, lifting the front of the thighs and making the skin much tighter. Subsequently, I liposhifted the fat underneath the skin to make it smoother, and finally cut the excess skin of the thighs through a vertical thigh plasty incision that is placed in the inner aspect of the thighs on each side. There we go.
So our patient is one month out from a lower body lift redo, buttock lift redo, buttock enhancement, thigh front lift, outer thigh lift, and vertical thighplasty. And she is eight months from a prior lower body lift that was done elsewhere. So I only have one really big question for you. What was the big difference? The skin was pulled up. I, um, all the loose skin in my thighs are gone. It's a major difference, night and day. Night and day difference. Mm-hmm. So the first surgery, was it hard to recover from compared to this one? This one was way harder. This one is harder, right? Mm-hmm. Yeah. And I ex-and I tell patients that these are harder. The way we do it is harder in terms of recovering because we are doing so much more work, and that's what makes it different, right? Night and day difference. Um, and that has to do with the fact that we've come, mobilized the, uh, tissues of the front of the thighs, lifted them, anchored them, done the same thing for the outer thigh, done the same thing for the butt, and then here we've done a th-vertical thighplasty or a thigh reduction. Look how smooth her thigh is. She's only one month out. She's not completely healed. She wants to go home, and I'm begging her to stay longer to heal, but, ah, you know, she's gonna do what she's gonna do. But look at these buttocks. And you can go back and look at her before. We ended up removing four and half inches of additional tissue from the buttocks. They are now perfectly round, right? I'm looking at her husband and I, I think they look phenomenal, guys. Just look. Let me rotate you. Just look at her profile. Look how beautiful she looks. And honestly, I'm as excited. I'm excited to see that she looks so amazing. Um, healing, still healing, and we still have things that we may have to do, but she got night and day change. Yes, absolutely. Right? You like it? Yes. Okay. Fine. Okay. Thank you. And she'll be back in a month from home, and then we'll do another follow-up video. Okay.
