High-Definition Lower Body Lift™ with Buttock Lift & Reshaping, and Spiral Thigh Lift - April 2025

SURGERY INFORMATION:

LEARN MORE ABOUT: High-Definition Lower Body Lift™, High-Definition Tummy Tuck™, Thighplasty, Buttock Enhancement

PATIENT AGE: Mid 40s

SURGEON: Dr. Siamak Agha MD, PhD, FACS

3-MONTH FOLLOW-UP TRANSCRIPT:

Following up on our patient. She's 90 days from this, and I can tell you her buttocks look phenomenal. Amazing. Gorgeous. Yeah. Right? Yes. So I'm really grateful that she's come all the way back from Washington for her follow-up. This is the only way that I can actually see patients. More than half of our patients come from out of town. It looks amazing, and she's healing beautifully.

This is her scar, and she was just telling me how invisible these scars are. And let me just pull it down, if you don't mind, for a second. See how perfectly they are placed. This was correction after liposuction elsewhere. She ended up with skin laxity of her thighs and her buttocks, so we ended up doing a buttock lift, buttock reshaping.

We liposuctioned her, did liposculpting, put some of the fat into the buttocks to create these beautiful buttocks, and then an outer thigh lift. And actually, we ended up doing not only an outer thigh lift, but also we lifted the front of her thighs and an inner thigh lift.

She had had a prior tummy tuck done elsewhere. I brought her incision lower in the front, cleaned her tummy tuck incision, and then performed an inner thigh lift. So her inner thigh incision is perfectly placed.

And then by virtue of doing this correctly, meaning that you want to put the inner thigh tissue behind the pubic fat, she still has fullness. A lot of times, I see these surgeries done incorrectly, and essentially, the pubic area and the thigh skin have been sutured like this, so there is no indentation in between.

What you want is the pubic area sitting over the thigh skin like this. You want to have that fullness, and we preserved that for her, and her incisions are perfectly placed. What do you think? I am very pleased with the results. I'm very pleased that I came here. Thank you.

Surgery Plan

SURGERY PLAN TRANSCRIPT:

So today we are doing a lower body correction and transformation. Our patient has previously had liposuction by a very good expert liposuction surgeon in Florida. Initially, she had calf liposuction, and her calves do look really, really nice. Subsequently, in 2015, she ended up doing liposuction of her thighs with a small amount of fat transfer to her buttocks, followed by recent liposuction of her thighs and lower buttocks and banana rolls that have resulted in skin laxity.

I have no issue with the surgeon who's done the liposuction because I know his work is really good. The problem here is the fact that she probably was not a good candidate last year to have a second attempt at liposuctioning her thighs and buttocks. Why? Because at some stage, when you continue to remove fat from your tummy or your thighs or any part of your body, especially as we age, you're essentially aging that region much faster, meaning that deflation of the thighs or buttocks or wherever you are at an older age by removing fat results in skin laxity, results in aging of the skin.

And the right thing to do is sometimes to tell the patients that, "You're not a good candidate for liposuction anymore, and if I liposuction you, you're going to get skin laxity." I have patients that come to my practice every day, and I have to tell them that, "I can't liposuction you. If I liposuction you, you're going to get skin laxity, and you're not going to like that. What we have to do is to cut you." And patients normally say, "Well, I don't want to have a scar." And then I have to say, "Well, I can't liposuction you either."

So she's one of those patients. It's not that the surgery was done incorrectly, it's just that she should have never had the surgery. She wasn't a good candidate. For her, unfortunately, that physician only does liposuction. So it brings me back to that educational video that I've made about don't go to somebody that only does one thing, because that means that physician is only trained to do that.

And most plastic surgeons are not trained to do just one thing. They go through six years of residency to be able to do a multitude of surgeries, reconstructive surgeries. So the training is extremely comprehensive, and therefore they can plan surgeries accordingly. But if you go to somebody that only does one thing and one thing only, then you're going to get that one surgery, whether you're a suitable candidate or not, you will get that.

And unfortunately for her, she had liposuction of her thighs, and as you can see, she's got contour irregularities of her thighs. She's got thigh laxity. And as we rotate her, she's got circumferential thigh laxity and buttock laxity. Keep on rotating, sweetheart. Okay.

So on her back, you'll see that she's got buttock deflation. She's been liposuctioned here. Again, buttock liposuction is not a good idea. You will get some skin laxity. And as we age, you get more skin laxity. So this is buttock aging, essentially. And then you see the banana rolls. And these are difficult areas to correct.

So for us to correct this, I have to do a buttock lift to really remove a substantial amount of upper buttock skin to lift this up, to reposition this, to put some of the fat back in the center of the buttocks. And then we have to reconstruct her infragluteal crease, lift this up like this and this, and do our spiral thigh lift procedure that we pioneered in 2012.

So make an incision, remove crescents of skin, and lift the back of the thighs, and then continue with that incision along her groin crease and do an inner thigh lift, outer thigh lift, and essentially a circumferential thigh lift. So that's the plan of the surgery today. We're going to be doing a circumferential thigh lift and a buttock lift for her.

Surgery Result - Back

TRANSCRIPT

So here you are looking at the back part of our surgery. As you may recall, our patient had had liposuction of the lower buttocks and her thighs, and then an energy-based device to tighten her skin, which has not worked. Hence, she's presented to me for a buttock lift, buttock reshaping, and circumferential thigh lift. As you can appreciate here, we performed the buttock lift, reshaping, augmentation with lower back fat pads, and additional fat transfer to the buttocks. And during this procedure, I created round buttocks, meaning that the width and the height of the buttocks are very similar, and therefore we have beautifully shaped and sized buttocks. And you can appreciate the fullness of these buttocks on the side view. You can see how beautifully round they have been remade. Coming to the thighs and the back of the thighs, I performed a posterior thigh lift. Posterior means back. This is part of our spiral thigh lift that we described in 2008. The most important part of this procedure is not only removing the excess skin that you have in the banana roll region down here, but also reconstructing this infragluteal crease, the crease that goes underneath the buttocks, meticulously. If you fail to do that, then the buttocks can appear elongated and get pulled down towards the back of the thighs.

Surgery Result - Front

TRANSCRIPT

So on the front, I continued with the thigh lift incision, brought it on the front aspect of the thighs, and performed an anterior thigh lift. And then incised over the groin crease, whereby she had had a previous inner thigh lift, and removed excess skin and scar from that region, and redid her inner thigh lift again because she did have some elements of labial spreading. So in this manner, she got a complete 360-degree circumferential thigh lift, lifting the front of the thighs, the outer thighs, the back of the thighs, and the inner thighs. And at the same time, we performed buttock lift and buttock reshaping. So a complete lower body transformation with special attention to the thighs and buttocks.
