PATIENT AGE: Mid 30s

SURGEON: Dr. Siamak Agha MD, PhD, FACS

3-WEEK FOLLOW-UP TRANSCRIPT:

So, a follow-up on one of our patients. Three weeks ago, I did a huge surgery on her. Total revision of virtually everything you see, including her thighs, including her lower body lift, including her vertical fleur-de-lis incision that I don't put on my patients. So we revised everything. We liposuctioned her. We sculpted her. We did a comprehensive lower body transformation.

She's five weeks from her wedding. So three weeks from surgery, and in five weeks' time, she's having her wedding. Tell me, what do you think? I am in awe. I feel like I'm shocked. I didn't think it could ever be me. Sorry, I'm going to cry because surgery makes you emotional. Oh, sorry. No, we're good. They're happy tears. They're good.

I never thought that I could have the legs that I didn't wear a skirt with. I haven't worn a skirt for the last 20 years, since I was a teenager. So the thought of getting to do that and to feel like all my hard work has paid off. Right. And to be able to see a muscle in the leg and not see the jiggle and to feel like I'm not wearing a jacket of skin. Yeah.

You can see how much we've been able to achieve with one surgery. Again, this is revised. I made her belly button smaller. I made this vertically oriented, meaning that her incision was crooked. You see how perfectly it's placed.

Coming to the front, she's healing three weeks, so you can see what a three-week incision looks like here and then here. We have one drain in place still. She's still draining a little bit. I like using drains because it gets my patients to heal faster.

These are her inner thigh, vertical thighplasty incisions. She had a ton of contour irregularities of her thighs that I corrected for her. But a lot of corrective surgeries went into her thighs, and then on her back, buttock lift, buttock enhancement, and again, let me show you scars and how perfectly she's healing and how perfectly these scars are placed.

So there you go. And you won't even know that she has had this lower body lift correction. This is a redo. Thank you for healing so well. Ultimately, that's half the surgery. Let's move. Dance with me.

Surgery Plan

SURGERY PLAN TRANSCRIPT:

So, a big surgery planned for our patient today. No pressure. She's only getting married eight weeks from now, so I don't know why we are doing this eight weeks before her wedding, but we are. Nevertheless, she's only waited 1,000 days for this surgery. I'm very honored and excited to do it, by the way, but we have big plans for her.

We're going to go after everything that hasn't been done correctly and remove all her graphic incisions and give her, hopefully, much nicer incisions. I'm not a fan of this surgery, but it's been done, so I'm going to redo it and make her tighter here. So we'll do her fleur-de-lis abdominoplasty.

We'll be lifting her thighs. We're going to go after all of these contour irregularities of her thighs that have been created with liposuction. We hopefully are going to do a thigh reduction. I say hopefully because I have to see how much liposuction I have to do, and if I do a ton of liposuction, I may not be able to do the vertical thighplasty comprehensively today.

Coming to the back, we're going to do an outer thigh lift, and then you see she's had a buttock lift, but look, she still has five, six inches of skin that we can remove today. So we'll be removing that. We'll be putting all this fat back into her buttock, creating buttock fullness with lower back fat pads. We'll be doing some liposuction.

So a lot is planned, and again, you have to do these surgeries comprehensively and correctly. Just cutting tissue out, what people call skin removal surgery, gives you skin removal. If you want to have shape, if you want to have the third dimension of projection, if you want to have curves, sculpting, you have to do that, and unfortunately, your average body lift doesn't do that.

Your belt lipectomy is a super basic body lift. It gives you the 360-degree incision, but it doesn't give you a high-definition body lift that is comprehensive, that goes after your tummy, your pubic area, your thigh front, your outer thigh, waist area, lower back, buttock lift, buttock augmentation.

It doesn't do that, and that is what you see on Instagram over and over when less qualified people are doing these surgeries because they are very comprehensive. This is probably one of the hardest plastic surgery procedures out there, but unless you do all the work, unless you reposition the fat and suture it at a higher position, unless you do the anchoring sutures, you're not going to get the results.

What you see today, what we're going to do today, is 20 years of me doing these procedures, leading the field. Our techniques are well established, and they're in plastic surgery textbooks, and even since then, I have evolved and made them better and better. So you'll see what we're going to do today. Great. I'm excited for her. Thank you.

Surgery Results - Back

TRANSCRIPT

Okay, guys. It's Saturday, and we are in the middle of her surgery. The back part of her corrective surgeries has been completed. The back part always takes a few good hours, and it's about half the surgery, and it's an area that many plastic surgeons don't really pay a lot of attention to in terms of buttock reshaping and enhancement. Our patient wanted fuller buttocks, so every ounce, every cell of fat was placed back into her buttocks to create roundness and fullness. We also did a flap augmentation of her buttocks with lower back fat pads. You see this crease on the outer thigh. This is an area that had been previously liposuctioned. I placed some fat in it, but unfortunately, I'm somewhat limited by what I can do there. I will try to add additional fat into the crease from the front. Hopefully, we'll get more fat. I've liposuctioned these areas. You can already see the shadow. Once the swelling goes down, we'll get a more hourglass look on her back. But already substantial correction has been performed, and it's time to turn her and continue with the rest of the work on the front.

Surgery Results - Front

TRANSCRIPT

So we are now finished with our surgery. It took me about eight hours to do a complete lower body transformation, revision, redo, and correction. She had surgeries elsewhere over the course of two years, and most of the surgeries were suboptimal, to say the least. Not enough skin was removed. She ended up with a fleur-de-lis incision, aggressive liposuction of her thighs resulting in internal scarring, depressions of the mid-thigh, too much fat in the upper thigh, too much fat around the knees, lots of skin laxity of her thighs in the front, in the outer aspect, and five to six inches of excess skin of the butt. So a lot was done, and a lot was done poorly. And not only did we have to revise and redo this and tighten her, I was faced with an anatomy that was already altered and deformed to some degree, which makes it even harder. Today, we were pulling up here. There were multiple vectors, a lift for the thighs, a reduction for the thighs, so they work against one another, and it makes the surgeries even more complex. And then liposculpting, creating a three-dimensional look for the buttocks, buttock projection. So a lot had to be done, and a lot had to be done at the same time to get the best results, but different vectors working in different directions. So every one surgery affects the next one. So you have to take all of this into consideration, and this is why not many plastic surgeons in the world can do these surgeries. These are extremely complex. In a nutshell, I did buttock lift, buttock enhancement, liposuction, flap augmentation of her buttocks, outer thigh lift, waist reduction, lower back skin excision, anterior thigh lift, which is the front revision of her tummy tuck. I tightened her abdominal muscles. I lifted her pubic area and removed some fat from it so that it ends up on a beautiful plane. Repositioned her navel higher by about one inch and liposuctioned the upper thighs, liposuctioned the knee areas, and fixed the thighs completely, and performed the thigh reduction, or a vertical thighplasty. And finally, you can see the precision in the placement of the scars, how symmetrical they are, and how perfectly they are placed despite the fact that this was a redo and the challenge was altered and deformed anatomy to some degree that we had to fix and work with.
