And the placement of the incision is not too low, not too high. She can wear a thong, and none of you will ever know that she has had a complete lower body lift.

I feel more comfortable in my own skin. Yeah. Yeah. After losing weight, you're still not happy because now you've got loose skin. Thank you.

Surgery Plan

SURGERY PLAN TRANSCRIPT:

Happy Thursday, everyone. Today, we are doing another body lift following yesterday's theme, but this one is a little bit different because she's already had one only four years ago. That's correct, four years ago. In 2019, she had a belt lipectomy in California, and unfortunately, she's not happy with the results.

And that has to do with the fact that a belt lipectomy, in my experience, and I've done probably more body lifts than most people on the planet, has to do with the fact that it's not the same as a thorough, comprehensive body lift, at least the body lift that we do. Many surgeons cut the skin and the fat underneath and suture the tissues together and close the incision.

As an example, you get an incision here, you get an incision here, the fat is removed, and this is closed. Okay? The problem is that all of the skin and fat that resides lower down is not elevated. When you cut the skin and fat here, this fat down here is still attached to the muscle here. So initially, it moves up, and it looks better because you're removing the excess tissue, but you're not elevating, you're not lifting the tissue.

So it's not a true body lift, and therefore, the phraseology of belt lipectomy actually suits that procedure. It just means that you're removing the lower abdominal skin and closing the incision, just like a panniculectomy. A panniculectomy removes the lower abdominal tissue and closes the incision. There is no repositioning of the tissue. There is no muscle tightening when it comes to a panniculectomy.

A belt lipectomy is like that, or a lower body lift that is not done correctly is like that, because excess skin and fat are removed, and the tissues are closed. It doesn't do repositioning. It doesn't do a true body lift.

In her case, we're going to be dissociating the fat and the skin from the muscle all the way down to here and then elevating it. So we are repositioning it. This is like a deep plane facelift versus a skin-only facelift. So when they came up with the idea of facelift, initially, they only removed skin. It never lasted, and therefore, over the past 50 years, facelifts have been technically advanced, and now we have deep plane facelifts, where we go under the muscle, where we go under the fat and elevate that.

This is a concept that we have pioneered since 2005. All my body lifts are repositioning body lifts, meaning that we go under, elevate, anchor, and then we close and remove the excess skin. And therefore, you get a much better and lasting result.

So these are her incisions here and here. We're going to be removing those. We're going to be tightening her lower abdomen, and we're going to do a thorough, comprehensive outer and thigh front lift.

And when we come to her back, her incisions are asymmetrical. You see, it starts up here and finishes down here. Keep on rotating, sweetheart. Let me get you 90, please. Give me one second. Okay.

When it comes to her back, you'll appreciate that her incision is slanted. Again, it starts close to this incision here and then ends up close to this lower incision here. So it's coming down. It's a straight, more or less straight line. We're going to change that. We're going to make it a beautiful V-shaped incision. We're going to demarcate the lower back from the upper butt, and our incision is going to finish at that junction.

We're going to be cutting more skin and elevating everything up again. Again, we would have to do this because it was a belt lipectomy. If it was done correctly, we would never have this much excess skin. The tissues were not repositioned, so that over time, the tissues pull the skin down, and you lower the scar and also create skin stretching, which is what you see. So you lose some of that effect and the result. So we'll be correcting all of those for her.

Surgery Results - Back

TRANSCRIPT

So we are now finished with her buttock lift, and as you can appreciate, her buttocks are now much rounder. Just compare the before with this. In order to perform a butt lift, what I do is to detach the buttock connective tissue and fat from the muscle, elevate it, and suture it at a higher position. And in this manner, we get a more lasting correction. With the reshaping, we come back and obviously use her own fat to shape and create fullness and roundness that is both vertical and horizontal. And then I performed an outer thigh lift for her, which will continue on the front. In total, given the fact that she had had a lower body lift only four years ago elsewhere, we were still able to remove a substantial amount of excess skin and restore and re-elevate her tissues.

Surgery Results - Front

TRANSCRIPT

So looking at the final result of our work in the front, our patient had previously had a lower body lift with a tummy tuck. So in the front, we didn't have to redo her tummy tuck. As you can appreciate, this tummy is somewhat flat and does not have the features of our high-definition tummy tuck, and that has to do with the fact that it wasn't done by us. It was done elsewhere. We did tighten the muscles of her lower abdomen here because there was a small amount of bulge. So we wanted to correct that as we were revising her abdominoplasty scar. We did a gentle lift of her pubic area, continued to lift the front of her thighs with the outer thighs, and then I did scar revision of her inner thigh scars. And as you can appreciate, she is much, much tighter. Her thighs are completely elevated and anchored at a higher position. So I expect our results to last much, much longer and hopefully will be permanent correction. There you go.
