SURGERY INFORMATION:

LEARN MORE ABOUT: Arm Lift , Breast Lift After Implant Removal
PATIENT AGE:
Late 50s
SURGEON:
Dr. Siamak Agha MD, PhD, FACS

SURGERY PLAN TRANSCRIPT:

So today we are doing an arm lift with breast implant exchange and breast lift. Regarding the arms, we're going to be doing an L brachioplasty with an extended incision that goes all the way from the elbow to the armpit, through which I'm going to... Let me rotate you. There you go. Lift the armpit higher like that, and she's helping me.

So don't help me, and then cut a lot of skin like that. She's going to have a nice tight arm. So this incision is in the front. This one is way in the back, and as they come together, we'll end up at the junction between the front and the back. So that's the arm lift, which will be the easier part of the surgery. Yeah. And then we'll do this arm is bigger, so we'll be cutting a little bit more from that one. Let me have your arms behind you. There you go. And then make them more symmetrical. Now, regarding the breasts, this is where the difficult planning comes into play. She wants an implant exchange and a lift, but it's far more complex than that because I have an implant that goes all the way to the midline here, both of them. Then they go all the way to her clavicle. That's her clavicle, but look at it. It comes all the way up there. It's down here now, but it comes all the way up there. It goes all the way out there, and it's low. So essentially, this is a big implant pocket with implants that are very mobile and asymmetrical, with this one being higher than this one. So it's not as easy as just going in there, doing a lift, and removing the implants. We have to now control this pocket because we don't want her to have this in the future, or this, or implant asymmetry, or implants that come underneath her armpit. So I have to close the pocket here, here, here, here, all the way down here. And then put a smaller implant because she wants to be smaller and do a lift. And then once I've achieved that, which is usually easy on one side, you have to do exactly the same thing on the other side, and that's not easy sometimes. Because a few sutures that are placed differently will change the whole equation. And you sometimes have to come back and take sutures out and redo it, and redo it till you get the final results. We also have nipple areola asymmetry, breast asymmetry, implant position asymmetry. So a number of factors that we want to sort out and fix to our best ability. And then we're going to be doing a little bit of liposuction here. I've done a lower body lift on her previously. It's going to be a little bit of icing on the cake for her, and that's it.

ONE YEAR FOLLOW UP TRANSCRIPT:

So we are one year out from a breast lift and arms, and as you can appreciate, our patient has healed beautifully. These are her breast scars with augmentation, and this is her arm scar. Looks really beautiful. We got a good amount of correction. We got an armpit lift through the extension of the incision and removing the armpit excess skin.

And on the back, she has no incision. Coming to the lower body lift, we can see her beautiful high definition result. And let me just try this out. This is her scar on this side. Again, in an underwear or a thong, you can't even appreciate the scar. Look at this beautiful navel and the fact that she's maintained her results one year out. She looks beautiful. Her abdomen looks defined, and she's working out, right? Yeah. Right. Okay. And then on the back, we just look at her buttock enhancement. She has scars, obviously. These are the scars for the buttock, perfectly and symmetrically placed. But when she wears a nice thong, her buttocks look phenomenal, to say the least. And one year out, so everything is permanent. And what do you think? I'm very happy. Very, very happy. Thank you.

Surgery Results

TRANSCRIPT

So you're looking at the final result of our work today. Today, I performed breast implant exchange with a breast lift and pocket correction, as well as an arm lift. Our patient previously had undergone breast augmentation with saline implants. Today, I removed the saline implants, which were about 600 cc each, and then replaced them with 400 cc implants since she wanted to be smaller. Also, the inner, the upper, the outer, and the lower pockets were all closed so that it can accommodate the smaller implant without moving too much. Obviously, you don't want to have an implant that can go from one side of the breast to the other, and unfortunately, her bigger implants were already doing that. So we had to do a pocket modification, make it smaller for the smaller implant, and then I performed a breast lift, and you can see how beautiful her breasts look now. And then we did an upper brachioplasty, removing a substantial amount of skin from both arms. The right arm was actually smaller than the left one, and we were able to match them both. And here we go. And the other side, there we go. And you can see how beautiful our incisions are, how symmetrically they have been placed, and I expect her to heal well and we'll hopefully get a great result.
