TRANSCRIPT

So today we are doing a lower body lift redo. Our patient lost about 115 pounds approximately 15 years ago and subsequently had multiple procedures back home on the East Coast. As you can appreciate, he has abdominal, body lift, chest, inner thigh, thigh reduction, and buttock lift incisions. He has essentially had a total body lift and is not fully happy with his results. He then found me, so now we’re going to revise the lower half for him, where he is bothered by several issues. What you notice is that we have a little bit of fat here, but that is not the issue. These are his incisions. The problem is that he has a lot of curves, and what we want, obviously, for a man is to create a more linear or V-shaped figure rather than saddlebags or too many curves. These areas haven’t been lifted enough, so I can go in, remove this skin, lift everything up, and really create a V-shaped result for him. Perhaps a thigh front lift wasn’t done, perhaps an outer thigh lift wasn’t done, or perhaps he had a circumferential abdominoplasty, which is very different from a lower body lift. People use the terms circumferential abdominoplasty, belt lipectomy, and lower body lift synonymously, but they are actually not the same thing. A circumferential abdominoplasty is more about the tummy and buttocks rather than the thighs. A belt lipectomy is similar in that it removes a roll of skin, whereas a body lift is really about lifting things up, securing them, and anchoring them in a higher position. Our surgical plan for him will be a complete lower body lift redo. He also has thigh reduction, or vertical thighplasty, incisions that are less than ideal, so we’re going to revise them and remove additional skin to create a better thigh contour. This is where I’m going to start the surgery first and then lift everything up. I’m going to perform a tummy tuck scar revision and pubic lift scar revision, and then start lifting the thighs upward. As we rotate him around, you can see the areas of fullness that should have been lifted higher. The buttocks are very empty, so although a buttock lift procedure was performed, the shape of the buttocks was not enhanced. There was no buttock reshaping and no buttock augmentation. We have an A-shaped buttock, which is typically what we aim to create for women, rather than the more square buttock shape we want for a man. We also have large areas of fullness that need to be corrected. By lifting, removing this excess skin, and repositioning the fat, we can enhance the buttocks, reduce these curves, and reshape his buttocks. I’m excited. I’m actually really excited to do this surgery. It’ll be fun.

Surgery Results - Thighplasty

TRANSCRIPT

I just finished our vertical thighplasty redo and removed the patient’s previous scars, as well as additional skin, to make the contour of the thighs more even. He had excess tissue in the upper thigh and around his knee, and both of those were corrected in this surgery so that now he has an evenly distributed thigh contour. Revisions are not easy because you’re dealing not only with excess skin and fat, but also scar tissue from previous surgery and, in addition, unevenness, and all of those have to be corrected as much as possible.

Surgery Results - Lower Body Lift

TRANSCRIPT

Today we performed a lower body lift redo on this gentleman who came from the East Coast. He previously had a lower body lift but ended up with more of a feminine, hourglass-shaped figure rather than a masculine figure. He presented with outer thigh laxity, saddlebags, buttock sagging, and deflation. Today we revised everything, including his vertical thighplasty scar from the previous surgery, augmented his buttocks with his own lower back fat and liposuction fat, and performed a complete lower body lift redo by elevating the outer thighs and correcting his overall shape so that we could create a more masculine figure rather than the curvier contour that resulted from his previous lower body lift.
