10-WEEK FOLLOW-UP TRANSCRIPT:

10 weeks ago, you had a lower body lift, right? I did. And she's now come back from Arizona for her follow-up. So tell me what we did. A lower body lift, which I'm thrilled with. It raised my hiney, and I have some shape here that I wasn't expecting.

Your amazing skills have given me, I think, already a beautiful tummy. Right. And my results are still showing every single day. I know that sometimes swelling can go on as long as three months, so I'll continue to change. It's improving. And it's getting better and better, right? Yes, absolutely.

May I show your scar? Absolutely. So this is her scar from the front. See how perfectly it's placed at the junction between her abdomen and, in here, the pubic area. The abdominal contour and the pubic skin are on one level.

And then as we rotate her, I'm going to rotate you this way, you can see the elevation of the thighs, or the thigh lift. Let's look at your back, or the hiney that you mentioned that you're very fond of.

And you can see her back and buttock enhancement. And this incision has been perfectly placed at the junction between her back and upper buttocks, and this is her incision.

So she's healing really well, 10 weeks out from a lower body lift. Thank you.

Surgery Plan

SURGERY PLAN TRANSCRIPT:

Today our plan of surgery is a lower body lift. Our patient is in her early 50s, and she's had a prior lap band placed. She's lost some weight and has recently gained the weight back. But nevertheless, she's ready to have a lower body lift to regain her old shape and contour. Many years ago, she also had a tummy tuck here.

So our plan is to go in and start with the back, but I'll just go over the plan in the front. When we come to the front, which will be after lunch, she's going to get a full tummy tuck with abdominal muscle tightening.

Her navel is off-center. I'm going to try to bring it to the center, and then I'm going to lower her incision by about an inch and a half while I'm doing a pubic lift.

Then, as we come towards the outer thighs and the front of the thighs, we're going to lift the front of the thighs, one of my own pioneering procedures, and then work on the outer thighs and the waist reduction. We're going to remove her old scar and give her a brand-new incision.

Her belly is not going to be absolutely flat because she does have a little bit of fullness here. And today we talked about needing to come back in the future and perhaps liposculpting that once she's healed from this surgery.

I can't combine that surgery into this surgery only because I'm planning to open this up, lower it, and suture it, and that interrupts the blood supply to the skin to some extent. It's safe. That's a tummy tuck. But when you combine it with a lot of liposuctioning, you're damaging your limited blood supply again, and in combination, it could be dangerous.

When people do liposuction and tummy tuck together, it's a different type of tummy tuck. It's a skin-only tummy tuck. Today we are not doing a skin-only tummy tuck.

On the sides, the plan is to complete an outer thigh lift. These are my conservative markings. We may be cutting a little bit more. I think we will be cutting a little bit more in her case.

And on the back, we're going to be doing a flap augmentation of the buttocks with fat transfer to the buttocks, a buttock lift, and we're going to be liposuctioning her mid-back rolls and her buffalo hump.

What I want you to also notice is that she's had prior work done here in this area. She's had liposuction, and I'll show you on the other side, too, and I will be correcting those for you. Okay?

And that involves trying to release some of these scar tissues and putting some fat into them for her so that we can really give her optimal results.

Surgery Result - Back

TRANSCRIPT

So we just finished the back portion of our surgery, whereby our patient got a nice buttock lift. You can see how beautiful these buttock cheeks look, nicely elevated, smooth, and rounded. Also, we liposuctioned her back to remove areas of fatty fullness. We ended up liposuctioning about three liters from her back and also corrected indentations on the sides that I showed in the video this morning. We removed some of the pre-axillary fat from the back, which we'll chase on the front next. As you can appreciate, she's got a beautiful back view, and now we'll be flipping her and starting the rest of her surgeries on the front.

Surgery Result - Front

TRANSCRIPT

So you're looking at our final results. On the front, I performed a complete tummy tuck redo with tightening of her abdominal muscles. Despite the fact that she had had a previous tummy tuck, her abdominal muscles were loose, and she had rectus diastasis, and I proceeded to repair that. In order to get there, I lifted the navel and floated it, and then created a tunnel all the way to her chest, and through the tunnel, tightened her abdominal muscles. We also ended up lifting the front of the thighs, as I do normally, and on the outer aspect of her chest, this area, if you remember, she had had depressions from her previous liposuction, and I placed about 50 cc of fat into each area.
