Happy Thursday, everyone. So today, we are starting the first surgery for our patient who is from Idaho and has lost 130 pounds. 130 pounds. And how much do you weigh now? 130. Okay, so she's lost almost 50% of her body weight, and she doesn't have much fat on her, but we have a tremendous amount of skin laxity and excess. So we'll be doing our high-definition lower body lift for her. On the front, removing all this loose skin, tightening everything up. As we come to her sides, we're going to remove, may I put your arms, nine and a half inches of skin from the hip region. You see all of this is going to get cut and make her tight. On the back, keep on rotating. We're going to do a bra line upper body lift so that I can correct this before I do a lower body lift. Typically, I do these on different days because truly it gives patients a better result when you actually separate them. But today, if I do my lower body lift now, I'm going to capture every wrinkle within my scar, and then she's going to have this permanently, essentially, built into the scar, which I do not want. So the first thing I'm going to do is to cut the excess skin to make this smooth, and then immediately do her lower body lift, removing nine and a half inches of skin from the hip area and the buttock area, and lift everything up and augment it with her own excess tissue. You ready? I'm ready.

So today I was planning to perform a bra line upper body lift for our patient, as well as a buttock lift. Normally, I don't do these procedures at the same time. I normally like to do the lower part first, give the patient time to heal, develop small amount of skin relaxation that typically happens with these surgeries, and then come and tighten the back again through the upper body lift incision and creating the best results for the patient. Given the amount of skin laxity that she had, I decided to do both of them at the same time today. But having said that, when I started with the buttock lift and buttock enhancement, I realized that I won't be able to cut as much from the upper back, and it may be prudent to wait and do that in the future. So here we are. We are now finished with our buttock lift and buttock enhancement and outer thigh lift, and I will be continuing with the patient's surgery on the front and in the future, we can go after the upper body another day. People often think that operating on smaller patients is easier, and that is true. It is easier to operate on smaller patients, especially if you're doing a skin-only body lift or a skin-only surgery. But our aim here is to transform and create shape and contour the patient. And in order to do that, in order to create concavity, in order to bring fullness here, in order to bring projection to the buttocks, I would need to be able to sculpt the patient to remove the excess fat and to use that to create the third dimension of profile or buttock enhancement. And I wish today we had more fat. I truly do.

So you're looking at the front part of our surgery, which included our signature high definition tummy tuck™. We essentially took that flat abdomen and created this silhouette with definitions, elliptical navel. We performed a pubic lift and removed staggering amount of skin from her thighs in continuation with the outer thigh lift from the back. There you go.
