Patient Details
This accomplished female patient in her mid-50s came to Dr. Siamak Agha after losing more than 200 pounds and previously undergoing lower-body contouring elsewhere. She had already transformed her health and weight, but the body she saw in the mirror still did not reflect the work she had put into herself.
In her own words, “Before I had had what I term as a skin removal, because there was really nothing done to make anything better.” She had researched Dr. Agha extensively before traveling to California and later said, “Nobody else that I’ve looked at has even come close.”
That distinction became the foundation of her Full Body Lift. Dr. Agha calls a transformation of this magnitude a body recreation. As he said while introducing her journey, “This is not just a body lift.” The objective was not simply to remove additional skin, but to rebuild shape, support, proportion, and the natural boundaries between the abdomen, buttocks, thighs, breasts, arms, and outer chest.
Stage One: Lower Body Recreation — Turning Skin Removal Into High-Definition Contouring
Her first stage converted the previous skin-removal result into a revision High-Definition Lower Body Lift™. Dr. Agha revised the lower abdomen and pubic area, lifted the front and outer thighs, reshaped the buttocks, and addressed extensive remaining thigh laxity.
One of the most challenging parts of her case was the previous thigh surgery. Her inner-thigh scars sat approximately two inches below the natural groin crease, while the scars beneath the buttocks also required repositioning. Dr. Agha removed and repositioned much of this prior scar tissue, reconstructed the groin creases, lifted the back and inner thighs, and rebuilt the creases beneath the buttocks so the natural borders of the lower body could be restored.
The buttock transformation is particularly dramatic. Dr. Agha described lifting the buttocks by approximately 5½ inches, reshaping them with her own lower-back fat pads, and adding 700 cc of fat to each buttock for additional roundness and projection. Rather than simply pulling skin upward, the tissues were secured in their new position. As Dr. Agha explained, “Everything you see has been anchored.”
The thighs underwent equally extensive reshaping, combining circumferential lifting with substantial liposuction and a revision Spiral Thigh Lift. Only weeks later, the patient said, “I’m absolutely amazed. I mean, it’s kind of mind-boggling. I don’t believe it’s me yet.” Later, looking at her dramatically smaller thighs, she added, “I never ever in a million years would have thought that these were my thighs.”
Stage Two: Completing the Full Body Lift — Breasts, Arms, Outer Chest, and Final Thigh Reduction
The second stage brought the upper body into proportion with the newly recreated lower body.
Dr. Agha performed an Internal Dermal Bra Breast Lift, internally supporting and repositioning the breasts higher on the chest while creating a rounder shape and fuller upper breast contour. A Total Arm Lift™ addressed loose tissue extending through the upper arms and armpits, while a J-Thoracoplasty continued the reshaping into the outer chest. A Vertical Thigh Lift completed the thigh reduction and further improved the dramatic lower-body transformation.
The final photographs show a body that reads very differently from top to bottom. Her waist is narrower and more defined. Her buttocks sit substantially higher with greater roundness and projection. The reconstructed crease beneath the buttocks creates a cleaner transition into dramatically reduced thighs. Her breasts now sit higher on the chest, her arms are slimmer, and the outer chest flows more naturally into the torso.
For the patient, however, the most important result was finally recognizing the body she had worked for. After describing her earlier result as little more than skin removal, she said of this transformation, “Now I look in the mirror and I’m like, wow, that’s awesome.” And later, directly to Dr. Agha: “You’ve changed my life.”
This case is why Dr. Agha uses the term body recreation. A comprehensive Full Body Lift after massive weight loss is not simply about removing what is loose. It is about rebuilding the shape underneath—restoring proportion, support, curves, anatomical creases, and definition so the outside finally reflects the extraordinary transformation the patient has already achieved.
