After achieving significant weight loss, this striking patient in her mid-50s was proud of how far she had come, but the loose and deflated tissues left behind prevented her body from reflecting that accomplishment.
Her concerns extended across the entire figure:
Because so many areas required meaningful correction, Dr. Agha designed a two-stage Total Body Lift rather than attempting to address everything in a single operation.
Stage One: Arms, Outer Chest, and Breasts
The first stage focused on the upper body.
Dr. Agha performed his Total Arm Lift, extending the correction beyond the upper arms and through the armpits to address the loose tissue along the outer chest. This created a smoother transition from the arms into the sides of the torso, rather than leaving behind a separate roll of tissue beside the breasts.
Her breasts were then rebuilt using Dr. Agha’s Internal Dermal Bra technique. Instead of simply tightening the skin, he reshaped and internally suspended her own breast tissue to create a higher, rounder, and better-supported breast mound.
Loose tissue from the outer chest was also brought forward and incorporated into the breast reconstruction. This allowed Dr. Agha to restore fullness using the patient’s own tissues rather than relying on breast implants.
Stage Two: Abdomen, Thighs, Hips, and Buttocks
Approximately eight months later, Dr. Agha completed the lower body lift transformation.
The loose abdominal skin was removed, and the separated abdominal wall was repaired from the upper abdomen down to the pubic region. The mons was lifted, while the front and outer thighs were elevated to reduce the downward pull affecting the lower torso.
From the back, Dr. Agha lifted the buttocks and preserved tissue from the lower back that might otherwise have been discarded. This tissue was advanced beneath the buttocks as natural augmentation, rebuilding projection with the patient’s own living tissue.
Fat transfer was then used to add further shape through the central and lower buttocks, soften the hip depressions, and restore additional fullness to the upper breasts.
The completed result is a more balanced and lifted silhouette from every angle. Her arms and outer chest appear lighter and tighter, her breasts sit higher with improved shape, and her abdomen is flatter with stronger waist definition. The buttocks have been elevated and restored rather than simply pulled upward, while the hips and thighs flow more smoothly into the torso.
Her elaborate floral and sea-life tattoo gives the transformation a final artistic dimension. In the before photos, the artwork rests over tissues affected by weight loss and gravity. In the after photos, the same tattoo now stretches across a more sculpted and harmonious form, as though the patient’s body has become both Dr. Agha’s work of art and a renewed canvas for artwork that was already beautifully her own.
