This lovely female patient came to Dr. Siamak Agha at The One Plastic Surgery Center in Newport Beach, Orange County, because her ears projected outward from the sides of her head. She wanted her ears to sit in better balance with her face without looking overly flattened, pulled back, or surgically changed.
For many otoplasty patients, the goal is subtle. The ears should frame the face naturally and no longer be the first feature people notice.

Dr. Agha performed otoplasty, also known as ear set-back surgery or ear pinning surgery, to bring the ears closer to the head while preserving a natural shape.
This type of ear reshaping is not just about moving the ears backward. The ear cartilage also needs to be shaped carefully so the result does not look flat, stiff, or overdone.
In this case, Dr. Agha created a more defined natural fold in the inner part of the ear using internal shaping sutures. These sutures help bend and support the ear cartilage, so the ear has a softer, more natural curve.
He also brought the deeper central part of the ear closer to the head. This helped reduce the outward projection while keeping the ear’s natural structure intact.
The result is a more balanced appearance, with the ears resting closer to the head and blending more naturally with the patient’s facial features. Instead of drawing attention away from the face, the ears now frame it in a quieter and more harmonious way.
A well-planned otoplasty should not make the ears look pinned, flattened, or artificial. For this patient, Dr. Agha improved the prominence of the ears while maintaining a natural ear shape, allowing her overall facial balance to come forward.
