This elegant patient in her early 70s came to Dr. Agha after previously undergoing a face and neck lift elsewhere. Although her first surgery had improved some aspects of facial aging, she remained unhappy with recurrent jowling, looseness through the neck, and visible platysmal bands beneath the chin.
Her case was more complex than a first-time facelift. Previous surgery had already altered the natural tissue planes and created internal scar tissue. She also had existing cheek implants, weakness of the right temporal branch of the facial nerve, and reduced movement at the left corner of her mouth before undergoing surgery with Dr. Agha.
Her goals were clear:
Dr. Agha performed a revision deep plane facelift together with a neck lift. Rather than relying primarily on pulling the skin, he carefully entered beneath the deeper facial support layer and released the retaining ligaments that were allowing the cheeks, jawline, and neck tissues to descend.
The deeper SMAS and platysma layers were then elevated and secured in a more youthful position. This allowed the overlying skin to settle smoothly over the newly restored foundation with less tension along the incisions.
The improvement can be appreciated throughout the face. Her jowls are substantially reduced, the jawline appears sharper, and the transition from the lower face into the neck is cleaner and more graceful. The cheeks appear elevated without looking overfilled, while her facial identity and natural expressions remain recognizable.
More than two years later, Dr. Agha performed a small secondary refinement. Fat was transferred to the hollow right temple and to an area of scar-related depression on the right side of the face after the tethered scar tissue had been released. A small amount of excess fullness was also removed from the upper cheeks using delicate micro-liposuction.
These refinements helped create greater balance between the temples, cheeks, and lower face without changing the character of the original facelift result.
The final transformation is polished but not overdone. She looks more rested, defined, and composed, with the lower face and neck once again supporting the elegance already present in her features. The goal was never to erase her age, but to restore the facial architecture that time and previous surgery had gradually obscured.
