This female patient came to Dr. Siamak Agha at The One Plastic Surgery Center in Newport Beach after a previous upper eyelid surgery performed elsewhere. Although that earlier procedure had removed upper eyelid skin, she was left with a hollow look above the eyes and a brow position that made the upper face appear tired.
Her concerns included:
Dr. Agha performed a temporal brow lift, a focused brow-lifting procedure that improves the outer portion of the brow through incisions hidden in the hair-bearing temple area. This helped elevate the frame around the eyes while keeping the result soft and natural.
To address the hollowing above the eyes, Dr. Agha also used a small amount of the patient’s own fat from the abdomen. The fat was carefully prepared into a fine graft and placed into the hollow upper-eyelid area, with 2 cc added per side. This helped restore subtle fullness without making the eyelids look puffy or overfilled.
The result is a more rested and balanced upper face. Her brows appear better supported, the upper eyelid hollowing looks softer, and her eyes look brighter without appearing surgically changed. From the side views, the brow position and upper eyelid area look smoother and less drawn, while the front views show a refreshed expression that still looks like her.
This case is a good example of how revision facial rejuvenation does not always require a large procedure. By combining a focused brow lift with delicate fat transfer, Dr. Agha improved the upper-eye frame and softened the hollowing left from prior eyelid surgery.
