

Patient Details
This patient in her mid-40s, came to Dr. Siamak Agha from Los Angeles, seeking revision plastic surgery after previous work in Beverly Hills. The patient desired a lighter, more naturally shaped result after noticing increased heaviness and breast ptosis (sagging) over her implants. She felt that the weight and forward projection of her augmented breasts no longer aligned with her comfort or aesthetic goals. The patient wished to undergo implant removal and a breast reduction and lift redo.
To restore balance, Dr. Agha removed her previous 375 cc smooth, round, moderate-plus profile Mentor implants, which had contributed to the downward pull of the breast tissue. He then performed a breast lift redo, employing the Internal Dermal-Bra Breast Lift Technique to reshape her breasts and uplift them again. This is an advanced breast lift technique that uses the patient’s own tissue to create internal support—functioning much like an internal bra. Approximately 345 grams of excess breast tissue were also removed from each breast to refine proportion.
To further enhance shape, the inframammary fold was elevated and secured to the chest wall to restore proper breast footprint, while outer chest tissue was advanced towards the center to narrow the breast base and naturally improve cleavage.
Her 1-year postoperative photos show beautifully lifted, proportionate breasts with a natural slope, centered nipple position, and a lighter feel that complements her frame.
