
Patient Details
This 64-year-old patient presented to Dr. Siamak Agha at The Aesthetic Centers in Newport Beach seeking correction after multiple prior breast surgeries that had led to breast deflation, malposition, and loss of support and shape. She had previously undergone implant removal with capsulectomy and a standard breast lift that resulted in near loss of the right nipple areola complex and poor healing. Having lost over 100 pounds, she also presented with significant laxity of the upper abdomen, outer chest, and mid back.
Her goal was to restore natural breast fullness and symmetry without implants while tightening the upper torso and lifting the breasts to a more youthful position.
To achieve this, Dr. Agha performed a comprehensive upper body reshaping procedure using two distinct upper body techniques — the Bra Line Upper Body Lift and his own J-Thoracoplasty technique — combined with inframammary fold (IMF) reconstruction, reverse tummy tuck, breast lift revision and breast augmentation with excess tissues from her midback, outer chest and upper abdomen.
In keeping with his surgical philosophy of maximal correction through minimal incisions, Dr. Agha designed the operation so that each incision connected seamlessly with the next. The bra line incision curved forward to meet the IMF, allowing every stage of the procedure, from upper back lift to breast reconstruction, to be completed through a continuous, concealed access line.
