
Patient Details
This patient in her late 50s came to Dr. Siamak Agha with significant breast asymmetry and discomfort after prior mastectomy reconstruction and multiple implant surgeries. She developed severe capsular contracture, implant malposition, outer breast indentation from thin tissue, and an oversized appearance from her previous 800 cc implants. She wished to achieve a smaller, softer, and more proportionate reconstruction.
Dr. Agha began by performing Bilateral Total Capsulectomy and implant removal, removing the contracted capsules and preparing the chest for reconstruction. He then reshaped the implant pockets by closing the lower and outer dimensions to restore proper implant support. He then reconstructed the loose inframammary creases on both sides internally, bringing additional support to the lower breast. Due to marked thinning on the right breast, an 8 × 16 cm Belladerm matrix was placed to reinforce the outer chest wall and improve long-term stability.
Next, upper abdominal liposuction was performed, and the harvested fat was processed and used for 140 cc of fat grafting to each breast—helping smooth irregularities, improve contour, and soften transitions. For final reconstruction, Dr. Agha selected Natrelle SRF-560 implants, a smooth, round, high-profile implant chosen for its ability to provide projection and support while complementing the patient’s reduced breast volume.
