This patient in her early 60s came to Dr. Siamak Agha at The Aesthetic Centers in Newport Beach seeking removal of her long-standing breast implants along with improvement in breast ptosis and asymmetry. Her goal was to transition to a more natural breast appearance without implants while still maintaining lift, shape, and support.
Dr. Agha performed a bilateral breast implant removal, explanting intact 330 cc saline implants from both breasts. To prevent deflation and sagging after explantation, he simultaneously performed his signature internal dermal-bra mastopexy—an advanced internal support technique that uses the patient’s own breast tissue to create durable structural lift. Multiple internal fixation points were secured to the underlying chest wall to recreate a stable, elevated breast foundation.
The nipple-areola complexes were precisely repositioned into a more youthful, balanced location as part of the structured lift. The breast tissue was reshaped and lifted to restore improved upper-pole support and a smoother breast contour without the presence of implants.
Her photos, taken approximately 12 months post-op, reveal restored breast shape, improved symmetry, and a naturally lifted contour without the added volume or weight of implants. The result is a lighter, softer breast profile with long-term internal support.
