

Patient Details
This patient in her mid-40s came to Dr. Siamak Agha after developing noticeable breast asymmetry from a previous saline augmentation—most prominently a deflated right implant, widened pockets, and lowered inframammary folds causing both implants to sit too low on the chest. She hoped to regain symmetry, improve implant position, and achieve a fuller yet natural-looking contour.
To correct these issues, Dr. Agha removed the previous saline implants and performed Bilateral Lower Capsulorrhaphy to elevate and restore the inframammary fold. This procedure involves closing the lower implant pocket on both sides. A Right outer Partial Capsulectomy was then added to widen the right breast and make it symmetrical with the left. This was followed by an Upper Capsule Release to reposition both implants higher and more proportionately on the chest wall.
The reconstruction was completed with Natrelle INSPIRA SSF-450 silicone implants—smooth, round, full-profile implants chosen for their enhanced forward projection and soft, natural feel.
Her 6-month Breast Implant Revision After results reveal a noticeably higher implant position, refined cleavage, restored symmetry, and a more cohesive breast shape from every angle. This case shows how targeted pocket repair combined with full-profile implants can produce a stable, aesthetic, long-lasting revision.
By correcting implant position, the level of the nipple areolae are also corrected on the patient’s breast.
